WHO WE TREAT

The Frida Center works with patients in the Portland, Oregon vicinity to confirm the diagnosis of fibromyalgia and identify the best treatment options for their specific symptoms. We also help those who have already been diagnosed to learn more about their illness and find better and more effective ways to improve their day-to-day pain and fatigue.

Due to the chronic nature of fibromyalgia and the need for ongoing management, we are unable to offer clinical treatment to those outside our immediate area. We plan to offer more educational resources for patients and clinicians in the future.

NEW PATIENT APPOINTMENTS

At your initial visit, you will meet at length with the provider, who will take a detailed history, confirm that the diagnosis of fibromyalgia is correct, and begin to develop a personalized treatment plan for you.

The first step is to complete our new patient appointment request form. We will then contact you to schedule an appointment. After we schedule you, we will send you our intake form to complete prior to your appointment. You can also download it here.

Follow-up visits will usually be scheduled for between two and four weeks after your first visit, and then as indicated in your treatment plan or just as needed.

For your first visit, please arrive a few minutes early and bring the following: your insurance card, a photo ID, a list of any prescription and over-the-counter medications you are taking (with dosages), any copay you may have, and your completed new patient intake form.

Please note that we charge $50 for any appointments cancelled without 24 hours notice.

PAYMENT

The Frida Center works with many insurance plans, including United Healthcare, Regence/Blue Cross, Lifewise, Pacific Source, ODS, HealthNet, TriCare/TriWest, PacifiCare/Secure Horizons and Medicare. Contact us to be certain we work with your plan.

If we are not contracted with your insurance carrier, your payment to the Frida Center can often still be reimbursed for part of the cost of the visit at “out-of-network” rates. For those paying directly or without any health insurance, we do offer a discount for payment at the time of service. Contact us for more details. Please note that we charge $25 for any returned checks.

REFERRALS

We do not require a referral, but some insurance companies do require a referral from your primary care provider for our services. Often we can help you determine if a referral is needed. Contact us for more details.

We work in coordination with your primary care physician and focus our care and treatment plan exclusively on fibromyalgia and related issues. Patients of the Frida Center are encouraged to have a PCP they can work with for other important health issues, such as diabetes, hypertension, high cholesterol, cancer screening and preventative care.
